Volunteer With the Festival

Erie Blues & Jazz Festival Volunteer Agreement

In consideration of my participation as a volunteer for the Erie Blues & Jazz Festival (hereinafter "the Festival"), I, the undersigned, hereby agree to the following:

1. Assumption of Risk

I understand that volunteering at the Festival involves, but is not limited to:

• Setup/tear down of equipment
• Lifting heavy items
• Working near electrical equipment
• Traffic management
• Cleaning

I acknowledge that these activities carry risks, including but not limited to slips, falls, injuries from lifting, and other risks inherent to an outdoor music event. I voluntarily assume all risks, known or unknown, associated with my participation.

2. Waiver and Release

I hereby release, waive, discharge, and covenant not to sue the Erie Arts & Music Festivals dba Erie Blues & Jazz Festival, its organizers, committee members, contracted employees, volunteers, staff, City of Erie, Department of Conservation and Natural Resources or Lake Erie Arboretum at Frontier Park (LEAF), and any sponsors (collectively, "Released Parties") from any and all liability for personal injury, illness, death, or property damage resulting from my participation, whether caused by the negligence of the Released Parties or otherwise.

3. Medical Treatment

I understand that I am not covered under any Workers' Compensation policy provided by the Festival. I am solely responsible for my own medical insurance and any medical costs incurred due to injury, illness, or death while volunteering. I release the Festival from any claim whatsoever that arises on account of any first aid or medical treatment rendered to me.

4. Voluntary Participation

I confirm that I am over the age of 18 (or have a parent/guardian signature), I am physically fit, and I am choosing to volunteer freely without coercion.

5. Photography Release

I grant the Festival permission to use photographs/videos of me taken during the event for promotional purposes without compensation.
